Home and Community Care (HACC) Case Manager
Northside Community Service is a not-for-profit community based organisation recognised for our expertise and innovation in Community Services, Community Development and Education & Care.
We are currently seeking applications from highly motivated candidates for the position of HACC Case Manager to join our Aged & Disability Services team.
The HACC Case Manager will be responsible for the effective and efficient delivery of individual support to HACC eligible clients who require assistance to remain in their own homes. The Case Manager will coordinate and deliver a suite of services to frail older people, people with a disability and their carers to encourage independent living and community participation.
You will possess:
* Demonstrated understanding of issues as they relate to frail older people, people with a disability and their carers.
* Relevant qualifications or experience working in Aged Care or Disability services.
* A high level of knowledge in the delivery of Case Management.
* Knowledge of services available in other agencies (government and non-government) and ability to assess and arrange the most appropriate services to meet client needs.
* Demonstrated ability to work independently and as a member of a multi skilled team without direct supervision.
* Sound understanding of ethics, professional values and boundaries and their application to the workplace.
* Well developed time management, communication, interpersonal, liaison and problem solving skills.
* Experience in record keeping, compilation of statistical information, preparation of reports and ability to assist in the development of policies and procedures.

This is a full time position with salary starting at $50,287.49 per annum.
