The Aged & Disability team at Northside Community Service is currently seeking applications from highly motivated candidates for the full time, fixed term contract position of a Case Manager, Outreach and Social Inclusion Services.
Reporting to the Program Manager, Outreach and Social Inclusion Service, Aged & Disability Services, the main duties of this position will be:-
. Assist participants of the NDIS through the 'phasing in' process
. Provide a strengths-based, person centred approach to case management, information, advocacy and referral by performing the following:
- Prepare & monitor support plans to meet the individual needs of clients
- Provide information, referral and advocacy support to clients
- Develop sustainable links with other agencies and organisations
- Develop comprehensive transitional plans for participants exiting the program.
. Manage a case load as approved by the Program Manager, Outreach and Social Inclusion Services in addition to developing a broad understanding of the circumstances of each participant in the program.
. Provide support to the broader Outreach and Social Inclusion team as and when required.
. Ensure program outputs and service user file notes remain correct and up to date.
. Provide monthly report to the Program Manager, Outreach & Social Inclusion Services.
. Active and consistent participation in team and departmental meetings.
