Tandem Respite Inc is a friendly and progressive organisation which provides
support and respite in home and in the community to the frail and elderly,
people living with a disability or mental illness.
We are offering you the chance to make a difference in people's lives by
joining the Tandem team as a Case Manager, a key link in efficient delivery
of support to Tandem clients who live independently in their own homes.
We are seeking highly motivated professional candidates for the position of
Case Manager to join our Service Delivery Team. As Case Manager you will be
responsible for the effective and efficient delivery of support to Tandem
clients to coordinate and enable independent living.
You will have qualifications in the social sciences or similar, and a high
level of knowledge in the delivery of Case Management and resources
available in local and extended networks. The rewards and challenges of
working within a complex and challenging service delivery environment will
be very familiar to you. You will have well developed communication skills
and be enthusiastic about working in partnership with all stakeholders.
