We are currently seeking a motivated, energetic and experienced person to fill the role of Transition Facilitator. You will work alongside school leavers to achieve employment, training, education, life skills and personal development goals by connecting them directly to the Canberra community.
This exciting role offers potential individuals an opportunity to work in an environment which is meaningful, satisfying, exciting and sometimes challenging as you support people to experience the best life possible.
Essential Criteria:

* Ability to work harmoniously as part of a small team

* Proven ability to connect with young people and implement strategies to engage them in the Canberra community

* Ability to network with the local community and relevant support providers

* Ability to develop positive relationships with families, teachers and other supporters

* Proven ability to work unsupervised

* Positive time management skills

* Current driver's licence

* Current first aid certificate

Desirable Criteria:

* Working with vulnerable people clearance

* Sound knowledge of ACT services and programs that promote community inclusion for young people

* Ability to implement ACT Disability Service Standards

* Experience in assisting people to; plan for the future and successfully implement their plans

* Experience providing skills development training and opportunities
