Just Better Care Canberra has created the Case Manager position in direct response to an increasing demand for services and a strong company desire to empower vulnerable people in our community to live independent, socially inclusive and meaningful lives. Working with a diverse range of clients in the ACT and surrounding regions, the Case Manager will be a committed, passionate and empathetic person possessing exceptional interpersonal and problem solving skills. This role requires a highly motivated professional, well presented, energetic, flexible and experienced within the community sector.
The role:
* Coordination of supports for clients under the NDIS requiring specific assistance to access therapies and specialised supports;
* Coordination of supports for clients with Aged Care Packages;
* Maintaining documentation and reporting procedures for NDIS clients and Aged Care Package clients;
* Monitoring of financial budgets and approved elements of NDIS clients and Aged Care Package clients;
* Ongoing support for general NDIS clients;
* Providing coordination support to general Just Better Care Canberra clients where required;
* Liaising with and building relationships with families and clients;
* Work as part of a team and provide administrative and coordination support where required.
Preferred skills / knowledge:
* Previous experience working in a Case Management role in Disability or Aged Care;
* Working knowledge of (or the ability to gain) NDIS guidelines and approved supports, especially in relation to coordination of supports;
* Working knowledge of (or the ability to gain) current Aged Care systems guidelines;
* A strong knowledge of support services that could assist and enhance the lives of clients;
* Ability to think beyond traditional approaches to support to find creative and personalised solutions for individual clients;
* Ability to develop and maintain relationships with families and clients;
* Ability to work autonomously and also as part of a team.
A current driver licence and reliable vehicle; excellent written and oral communication skills; excellent computer skills and the right to live and work in Australia are essential.
Applicants must hold a current National Criminal Police Check, a Working With Vulnerable People check and a Working with Children check. Qualifications in a relevant field are highly regarded.
This is a permanent position with weekly hours negotiable from 3-5 days per week. There is the possibility of working remotely for the right candidate. The position is based in Fyshwick, ACT.
