The 5th Annual ACT Disability Support Worker Awards - 2012

Purpose of the ACT Disability Support Worker Awards

The ACT Disability Support Worker Awards recognise the achievements and
valued role of direct support workers in the A.C.T disability service
field. The awards celebrate the contribution of disability support
workers and their professional relationships with people with a
disability, their family members and friends, peers, other service
providers and community networks.

Who is eligible for an award?

In 2012 there will be five awards

1. Best individual support worker minimum two years as a support
worker

2. Best team leader provides direct supervision to disability support
staff

3. Team of the year a group of support workers providing direct
support

to a person, or discrete group of people, with disability

4. Rookie of the year between one and two years as a support worker

5. Long-service award more than ten years providing direct support

Nominees may support clients in a residential, community, advocacy or
employment setting.

To be considered for an award the nominee/s will be:

P Recognised as valued and highly respected in their field of work.
P Employed in a territory funded or operated disability service (Note
this usually precludes staff working in psychiatric, kindergarten,
school and higher education/TAFE settings).
P Working in a direct support role (not working as a health
professional)

How to lodge your nomination

The hard copy of the nomination form including attachments must be
submitted by post or fax - please also send a back up electronic copy to
DPLN@act.gov.au<mailto:DPLN@act.gov.au>. Nominations close on Friday 2
November 2012. Remember you may also include supporting documentation
such as letters and references and you must sign your name on the form.

Who can make a nomination?
Nominations will be accepted from peers, managers, and people with a
disability, their family and friends.

Award Ceremony

A cocktail party and award ceremony will be held at the Southern Cross
Club Woden, Corinna Room 2 Thursday 29 November 2012, commencing at
5:30pm. All award nominees will be notified and invited to attend the
awards ceremony. Each nominee (including every member of every team)
will be entitled to invite one guest at no cost. Extra tickets will be
$20 per person.
