Contract Position: Accommodation Programs Case Worker
The Disability & Aged Care Services is seeking an energetic and passionate
individual to backfill 7 months of maternity leave in the Accommodation
Programs.
This is contract position starting from the 9th of September, and includes a
handover with the current worker to assist with continuity of support.
The Accommodation Case Worker is responsible for working with the
individuals and families in the TWIN (Towards Independence) and ISP
(Individual Support Package) Programs within the portfolio.
As the Accommodation Case Worker you will work within a case management
framework to support adults with an intellectual disability who live in
independent or shared accommodation within the Canberra community. A focus
of these programs is supporting and promoting the independence of
individuals through skill development and enhancement of quality of life
through increased access and connectedness to their local community.
With the implementation of DisabilityCare Australia (the NDIS) approaching,
this position will require you to work closely with individuals and their
families to facilitate greater choice and control within the current system,
while also supporting individuals to begin thinking about their future goals
and aspirations under a self-directed funding model.
This role provides support to the Disability Support Workers who work in the
field, participates in direct support shifts and in On Call as required.
This is a full time position and is paid at a Level 5 of the CatholicCare
Enterprise Agreement.
