Whatever you call it, it is part of ADVANCE HEALTH CARE PLANNING
If you are interested in learning more about opportunities for choice and control in this aspect of your own health care or the health care of loved ones or clients, please come and learn more at one of the following information sessions:
Tuesday 31/3/15 10-12 Mental Health Community Coalition Training Room Level 1 Griffin Centre Genge St, Civic
Thursday 2/4/15 10-12 'Shout' Small Meeting Room Bldg 1, The Pearce Centre, Collett Place, Pearce
These sessions will be followed up a week later with 'Paperwork Clinics' for anyone who would like to complete the documents and may feel the need of more information or support.
