PATH: Are you trying to get ready for the NDIS? Do you need help with planning?

Need help with planning?  Book your PATH now!
With the recent introduction and phasing in of National Disability Insurance Scheme, useful resources and tools to assist people with their life planning are essential.  Do you need help to plan and achieve your goals?  Do you need to start preparing for your NDIS assessment?  If so, the PATH planning process may be the tool you are looking for to help you to get started!

PATH is a visual planning tool that helps people with disability to plan effectively for their future.  A PATH plan can enable a person (and their family and supporters) to identify steps towards achieving their goals. The PATH plan can be very useful in helping people to focus on what is important to them in preparation for their NDIS Planning sessions.

PATH is suitable for people of all ages, and all disabilities.  The PATH facilitation process takes approximately 3 hours to complete.  Two trained facilitators work with the person with a disability, and their invited participants, to complete the PATH plan.

We have an expert team of facilitators who are available to guide you through the PATH planning process.

Please note:  Minister Burch recently announced the new 2015 NDIS Community Participation Grants to assist people with disabilities plan for the NDIS, and the good news is that both PATH and it's implementation is eligible for funding.

If you are eligible for an 2015 NDIS Community Participation Grant, the cost of having your PATH completed is $400.  However, we do offer a subsidised concession rate of $100 for a PATH if you are not eligible for one of these NDIS grants.  Please note that our subsidised rate is only available until the 30 June 2015.
